ASA Crosswalk Updates for Anesthesiologists

Below are the most recent updates for physicians, with Crosswalk guidance from the ASA. There are updates to Crosswalks for two CPT codes:
- 45378 – Colonoscopy, flexible, diagnostic, including collection of specimen(s) by brushing or washing, when performed, separate procedure; and,
- 45330 – Sigmoidoscopy, flexible, diagnostic, including collection of specimen(s) by brushing or washing, when performed (separate procedure).
The 2018 Crosswalk entries for these procedures list code 00812 (screening colonoscopy) as the primary anesthesia code option and 00813 (combined upper and lower GI endoscopy) as an alternate code. Here is a clip from the ASA release:
The ASA Crosswalk Editorial Panel notes that in some instances it may be appropriate to report anesthesia care for these procedures with 00811 (a colonoscopy not designated as a screening nor performed with an upper GI endoscopy).
Zotec Partners was concerned with the original guidance as soon as it was released in the 2018 Crosswalk. We reached out to the ASA immediately with our concerns as we knew there were many instances where 00811 (Anesthesia for lower intestinal endoscopic procedures, endoscope introduced distal to the duodenum; not otherwise specified) would be appropriate to report when the surgeon would be reporting 45378 or 45330 for his/her services. Our biggest concern included what a payer would do in the event of a review – noting that 00811 was not an option for the two CPT codes in question. We are pleased that the ASA has issued this update in a timely manner, and we are committed to keeping you informed with the latest information available.
